Nutritional profile of people with disabilities and the high prevalence of obesity in Down syndrome and in the female population

Abstract

Health care should include a global care with nutrition and food for the population, including those with disabilities. Multidisciplinary performance is essential for improving the quality of life and the performance of health professionals should be based on the recognition of the characteristics of the target population. People with disabilities may subject to metabolic changes, eating limitations, drug interactions and side effects, factors that interfere on their nutritional status and/or may require special nutritional care. The evaluation of nutritional status aims at the diagnosis of the population in order to outline strategies for the treatment or prevention of health problems related to food and nutrition. The present study had as objective to know the nutritional status of population attended in school for people with disabilities through anthropometric measures as the initial stage for the realization of education interventions and nutritional attention to this population. Material and methods: students, interior of São Paulo state were evaluated, including every students except those are bedridden and wheelchair. Children, adolescents and adults (7-49 years old) with different disabilities were evaluated for body mass index (BMI) and arm fat area (AFA). For these indicators, measures of weight, height, arm's circumference and skin fold of arm were obtained. The evaluation of BMI of adults and the weight to age of children and adolescents were made according to WHO and NCHS, respectively. For Down syndrome (DS) patients up to 18 years of age, the Cronk's growth curve was used. Results: a total of 111 disabled people, both sexes and between 7 and 59 years of age, were evaluated, with the majority (53%) being adults, and females (64%), and DS being 11%. According to the anthropometric data, 74% presented some nutritional problem, highlighting the malnutrition (26%) and the obesity (20,7%). The comparison of the nutritional status between individuals with and without DS, indicated that there is high prevalence of obesity, while malnutrition affects non-carriers. The prevalence of overweight is also higher among women than for men, a more evident observation when in the DS situation. The arm fat mass was also greater among people with DS. Conclusion: The nutritional board of the disabled persons from a teaching entity is worrying since there are prevalence of extreme situations as low and overweight. In people with DS, obesity is the most prevalent nutritional state, and it was more evident among women with DS. The study allowed to obtain data for recognition of the population and its nutritional characteristics. Overweight and malnutrition require actions to improve health and quality of life, such as integral care of the person. The data of the nutritional profile are essential for the performance of an intervention activities, with strategies appropriate to the target population, and prevention or treatment of nutritional disorders.

    Professor assistente doutor atuando na área de Nutrição, sendo uma das áreas de ação Nutrição em saúde Pública, Avaliação nutricional de populações, Epidemiologia da nutrição, as quais este trabalho está vinculado.

    Docente do Departamento de Educação do Instituto de Biociências de Botucatu, da Unesp. Responsável por disciplinas de graduação e pelo laboratório didático  denominado CEPRAN , Centro de Estudos e Práticas em Nutrição, onde estagiários realizam ações com a comunidade em orientação, avaliação e educação nutricional, com atendimentos nutricionais e multiprofissionais. As pesquisas atuais envolvem associaação entre consumo de alimentos funcionais e consumo de alimentos fonte de micronutrientes e obesidade ou doenças crônicas não transmissíveis. Estes trabalhos vem sendo realizados em população adulta ou em crianças na comunidade e atendidas no CEPRAN.
